Basically, she discovered that she only existed in the Otherworld because she was dead, and her spirit had been corrupted, just like the Puppet Nurses, and through that realization, she became a monster, just like the others.

The original version of the game (as in, the original devs) simply botched all of the original SH1 characters and the newer devs who made the retail version tried the best they could to correct that but there's only so much they could do. It's why you notice so many inconsistencies with them.
